Forms for EBS Applications

EBS Licensees are required to file online to update administrative information, renew the EBS license, enter ownership disclosure information, process assignments, and apply for excess capacity airtime (spectrum) leases. Filing is accomplished on the Universal Licensing System. The required information is entered online and the ULS uses that information to complete the form. A copy of the form can be viewed and printed when the filing is complete.

The FCC provides copies of the forms on its website. The downloadable forms can be printed out for reference purposes and to review before completing the form online.

Purposes of the Form:

AU – Administrative Update

Changes limited to the following: Licensee Name (without a change in ownership) Entity Name (without a change in ownership, control or corporate structure), Mailing Address, Telephone Number, Fax Number, E-Mail Address, Contact Information and Control Point Information

CA – Cancellation of License

DU – Duplicate License

RO – Renewal Only
