Switch Communications Group LLC (the world's highest density data center) has provided a map of fiber optic broadband showing the routes of 26 national fiber optic carriers. The interactive map on the Switch website allows you to click on a company logo to display its fiber optic route. (The routes of major telecommunications companies, such as Verizon and AT&T, are not displayed individually.)
